HEALTHY BEGINNINGS

 

MEDICAL INFORMATION                            

 -you need completed physical examination form, immunization dates, proof of lead screening compliance, dental screening and vision testing results.

-please notify me of any food sensitivities or allergies. Also any other severe allergies, i.e. bees, etc.

-ALL MEDICATION must be administered by the nurse and must follow the school policy- you need a written doctor’s order- please contact me to discuss this and if you need an order form.

- let me know if your child is taking a daily medication at home so I may be aware of any side effects.

-please be sure your child's emergency card lists people who can pick him/her up if they are ill and I can't reach you. Also update your phone numbers when they change.

-if your child has to be excused temporarily from gym class and recess for a medical reason, we will need a doctor’s note (please note- even if the doctor writes “No Gym class”- it will also include no recess as well, for your child’s safety)

 COMMUNICATION

- please don't hesitate to call me about anything, and be sure that I will call you if I have any concerns. If I’m not in my office when you call, you can leave a phone message. Unless it’s urgent, I might not be able to return your call until after school dismissal.

- if your child comes to see me during the school day for a problem, I might send a note home with them if it’s more than a minor scrape or complaint. I also might call you at home, work or cell #.

-if your child has an infectious disease please let me know so I can alert the other families in the classroom of the exposure.

- alert us of any big changes that may be going on at home, as this often will be demonstrated with "I don't feel good" when they’re in school.

GENERAL WELLNESS

-  Bedtime should be early; it helps to start the routine a week or two before school starts. Your child should get at least 11 hours of sleep (ages 3-5).  School aged children Grades 1-5 should get at least 10 hours of sleep.

-allow them enough time to get up in the morning and get organized, and most importantly, have breakfast

- please send healthy snacks and send one every day

- teach them how to open juice boxes and pre-packaged snacks

- teach them to cough, sneeze into their inner elbow/sleeve area; use a tissue when they sneeze, & then wash their hands.

  -work with them on verbalizing what it is that they need

-have them attend to buckles, zippers and buttons by themselves when using the bathroom

- explain modesty to them

- please keep them home if they’re ill. Notify the school if they are absent and especially if they have a contagious illness, i.e. strep throat, conjunctivitis, impetigo, etc.   

- it might be helpful to pack spare underwear, pants and socks in a large Ziploc type plastic bag and keep it in your child’s backpack, in case they have a bathroom accident, gets wet or spills their drink.

I want your child to have an optimal learning experience. Their good health will be an important part of that.  Please let me know if there is anything I can do to make that happen.
