Section 1: White “Everything Else” Log

This page is a like a table of contents of all of your tasks that are not
ELA, Math, or Science
-
List them and include the LE’s and GSE’s. Reflections are not necessary for
this section.

Only the student’s signature is required on this page.

Section 2: Yellow ELA Log
-
You must list at least 7 ELA tasks
-
List the LE’s and GSE’s for each task
-
Your advisory teacher must initial each task. This means you have shown
your advisory teacher the cover sheet, student work, rubric, and
reflection (all must be included) for each task
OR
you have record on your RIEPS print out that your task is rated
either a 3 or a 4(status doesn’t matter).

All tasks must have a reflection.

Both the advisor’s and student’s signatures are needed on this yellow sheet.

Section 3: Green Math Log

You must list at least 7 math tasks
-
(Follow the same process as the ELA log)

Section 4: Orange Science Log

You must list at least 6 science tasks

(Follow the same process as the ELA log)

To exceed the standard, you must have more than the 7 ELA, 7
Math, and 6 Science tasks.
ü
All tasks MUST meet standard or better.

Section 5: Blue Learner Expectation
Sheets

Students should fill out the information at the top of the blue sheet

You do not need to indicate a score on the rubric.

If a reflection has already been written for the task or the task has been
rated either a 3 or 4 on the RIEPS print out, you do not need to
rewrite the reflection.

For each Learner Expectation you must include:
o
Cover sheet
o
Student work
o
Rubric
o
Reflection
(either a previously written one or complete the back of the
Blue Sheet)
Or

If your task has been rated on the RIEPS system, you must include a copy of
your entire RIEPS printout (with your name on the first page) and highlight
the appropriate task.

An advisor or a teacher must sign the blue sheet to verify that the task
matches the Learner Expectation.

Any tasks not being submitted as a Blue Sheet but are on the
proficiency logs can be kept in the advisory file cabinet for storage.

You may use your Capstone certificate as ANY Learner Expectation – MUST use
Capstone certificate.
When choosing your 9 tasks to match the Learner Expectations, consider the
following questions:
What is your goal?
Which tasks best represent you as a learner, your interests, and your
abilities?

Upon completion of your work, the advisor completes the 2010
Graduation Portfolio Checklist. This form will go into the front
pocket of the binder.

The portfolio is then submitted to the PBGR office by the student
and will be scored and returned to the advisor.
